Full time Hospice LPN
General Summary: LPN will be responsible for the management of care for assigned patients under the supervision of an RN using an interdisciplinary Team approach. The LPN will perform duties according to HPCT policy and procedure; and the scope of practice as defined by the Virginia State Board of Nursing. LPN must be willing to work on a scheduled weekend rotation.
QUALIFICATIONS: Must have strong clinical skills, excellent verbal and written communication skills. Excellent customer service and computer skills a must. the LPN must be currently Licensed and in good standing with the Virginia State Board of Nursing.
EDUCATION: LPN shall have a least 2 years experience in nursing as a LPN and at least a minimum of one year experience in hospice or long term care. Must have desire to learn and demonstrate aptitude for the hospice philosophy and palliative care is required. Must have a valid VA driver’s license.
Duties will include but not limited to: Triaging of patients/family calls, making SN visits to patient’s home/facility as appropriate, documenting all calls and visits. Provide hands-on clinical nursing within the LPN scope pf practice. Assist with planning, implementation, evaluation and recording of client teaching and outcomes of the Plan of Care. Participate in Interdisciplinary Team meetings and assist in up-dating the Plan of Care for assigned patients based on identified problems and needs as appropriate. Provide care to manage patient symptoms. Work as a Team Member at all times and uphold the Hospice Code of Ethics, Traveling required within Hampton Roads Area, reliable transportation and car insurance required. Mileage reimbursed. RN – Case Manager
Hospice and Palliative Care of Tidewater is seeking a Registered Nurse from an accredited program currently licensed and in good standing with the Virginia State Board of Nursing. Must have at least 1 to 2 years’ experience in nursing in an acute care setting or 1 year experience in home health, hospice or comparable experience is preferred. Desire to learn and demonstration of aptitude for the hospice philosophy and comfort care required. This position offers flexibility, potential for growth, a positive working environment, and an opportunity to interact with patients and their families on a more personal level. Excellent customer service and computer skills a must.
Duties will include but are not limited to: Conducting regular visits at home or nursing facilities for a caseload of approximately 10 to 13 patients ; Coordinating, assessing, and implementing patients’ care; providing accurate and timely documentation of patient services to reflect plans of care; overseeing and participating in day to day clinical care of terminal patients and their families; attending interdisciplinary meetings; interacting with other group members such as CNA’s, LPN’s, Social Workers, Chaplains and Volunteers; rotating on-call on an as needed basis.
Full Time CNA
General Summary: responsibilities will include but are not limited to:
Assist with personal care of patient, i.e. bathing, feeding, bed linen care, etc., etc. Document in patient’s chart. Assist with teaching patient/family concerning patient care and care for the caregiver. Bereavement activities as requested and/or able. Maintain appropriate amount of supplies at patient’s residence. Keep DON/RN Case Manager informed of condition of patient/family. Provide emotional support to patient/family. Attend all staff meetings. Traveling within the Hampton Roads area.
Qualifications: MUST HAVE computer experience, excellent verbal and written skills, excellent customer service. Candidate must embrace the hospice philosophy. Valid driver’s License an reliable transportation a must.
Education and Experience Requirements: Candidate must be a graduate of a State Certified Nurse’s Aid program and in good standing with the State Board of Nursing. Required education: Associate – Required license or certification: CNA

Hospice and Palliative Care of Tidewater is seeking a certified nursing assistant to provide care for hospice patients.
Job requirements include: ability to read and follow instructions, document care given, demonstrate tact and patience, traveling within Hampton roads area, possess and maintain current driver’s license, auto insurance, dependable transportation, and experience with electronic medical records. Previous home health or hospice experience is preferred.
Visits are paid on a per diem basis. Working hours are normally from 7am – 3pm.
